    COD All accounts Not Pre Approved NET 30

    All accounts not paid by terms or over the credit amount will be COD until until financial arrangements are made and satisfied.

    Service charge is computed on a percentage rate of 1.5% per month which is an annual rate of 18% on any previous balance not paid.

    for and in consideration of extending your credit at my request to Lund Oil, Inc. hereby personally guarantee the payments to Lund Oil, Inc. of any obligation of the company. I hereby agree to bind myself to pay Lund Oil, Inc. on demand any sum which may become due to Lund Oil, Inc. by the company whenever the company shall fail to pay the same. It is understood that this guarantee shall be continued and irrevocable guarantee and indemnity for such indebtedness of the company. I do hereby waive notice of default, nonpayment and notice hereof and consent to any modifications of renewal of the credit agreement hereby guarantee.

    I hereby declare that all of the statement made in this application are true and are made for a purpose of obtaining credit. Lund Oil, Inc. is authorized to investigate my credit to verify to obtain such other information as the lenders deem necessary and to give credit reporting agencies and other information regarding the lenders credit experience with me.
